Uninsured Services 

As family doctors, we are committed to helping you with all of your health care needs.  As you know, the Ontario Health Insurance Plan (OHIP) pays for most of your medical visits here at the office, however, some medical procedures and most administrative services are not insured by OHIP.  These services are called “uninsured service”. These services are still provided by this office but require significant time and resources to administer.  In the face of rising office expenses and government restrictions, we are no longer able to provide these services to you free of charge.  In order to maintain the highest standard of care, payment for these services have become the responsibility of either the patient themselves or the agency requesting the service.

Your doctors will charge a fee for uninsured services in accordance with the fees guidelines set by the Ontario Medical Association (OMA) and the College of Physicians and Surgeons of Ontario (CPSO). Please see below for a list of uninsured services fee guide. However, if the service you request is not on the list, please communicate with your doctor or the clinic’s staff and we will provide further clarification.

 

 

List of Uninsured Services Fee Guide

 

Intermediate visit (without valid health coverage)--$80 on weekdays, $120 on weekends/holidays

Physical exam and form when requested by 3rd party--$130

No-show fee: regular visit--$30, periodic health visit--$60

Replacement of lost immunization record, referral note, lab requisition or prescription --$20

Prescription refills by phone/ fax (when appropriate and only at the request of patient or patient’s representative)-$20

Copying –$1/page up to 30 pages, $0.25 /page thereafter

 

Back to work/school note, sick note, daycare note (free from communicable disease) --$25

Forms for school, camps, preschool, daycare, university or other educational institution (excluding examination)--$25

Forms for insurance coverage of massage therapy, compression stocking, orthotics, etc.--$20/each

Medical certificate for employment insurance maternity/sickness benefit INS5140--$35

Medical certification/form of pre-employment, fitness club, hospital or nursing home employee--$35

Medical certificate for employment insurance compassionate care benefit-- $50

Jury duty note or letter--$50

Travel cancellation insurance form or letter--$40

TB skin test for employment purpose-- $40/1 step, $60/2 steps

 

Driver’s Medical Exam and form--$150

Life insurance death certificate--$50

Revenue Canada, Federal Disability tax credit--$50

CPP disability medical report form (form T2201)--$85

 

Pre-travel counseling and prescription--$40

Travel related vaccine injection (HepA, HepB, Twinrix, Typhoid, Menactra, etc.)--$20/each

 

Transmission of medical records --$50
