  RE: Periodic Health Visit

 Dear Patients:

There is evidence that annual physical examination is ineffective in finding hidden disease in healthy people. Starting January 2013, the annual physical examination has been replaced by the Periodic Health Visit for patients with no apparent medical problems based on the 2012 Physician Services Agreement with Ontario of Health and Long-Term Care (MOHLTC).

We will now be performing Periodic Health Visit at specified intervals. Periodic Health Visits are office visits for healthy patients who have no apparent medical problems, which are used primarily for screening and promoting healthy behaviours. The physician and patient can use the appointment to discuss screening for cancers and other preventative health issues relevant to the individual patient’s medical history and lifestyle. In addition, any examination required will be focused on the individual needs of the patient relevant to their risk profile depending on their age, sex and health history.

These are performed at time frames which mirror when certain screening tests are performed. These visits are required less frequently when you are young, and more frequently as you age. The Ministry of Health of Ontario is in support of this transition. Generally, Periodic Health Visit occurs every 1-2 years for men and women older than 65, every 2-3 years for men and women of 50-65 years old, and every 3 years for women and men under 50. The interval may be adjusted by your doctor depending on your medical circumstances.

Many patients will still need regular office visits to assess and monitor their medical conditions like heart disease, diabetes, lung disease, liver disease and many other conditions. These "Chronic Disease Visits" are often every 3 - 12 months. If you have a concern or multiple concerns which you feel will require more time, then notify the receptionists and they will work with you to get the right visit type and time to address your medical needs.

Remember to bring a complete list of medications you are currently taking, a summary of your health history, and any questions you have for your doctors to every Periodic Health Visit.
